Transition to Adulthood Assessment for Young People in Out of Home Care

The READY Futures Program is a specialised occupational therapy transition assessment for young people aged 15–17 living in Out of Home Care, designed to support preparation for increasing independence and adult life.

Delivered across multiple sessions, this comprehensive assessment evaluates a young person’s daily living skills, executive functioning, community participation and readiness for adulthood, helping identify both strengths and areas where additional support may be required.

Balance to Bloom provides READY Futures assessments across the Perth region and throughout Western Australia.

How the READY Futures Assessment Works

The program is completed across structured occupational therapy sessions and may include:

  • Real-life functional skill observation in home or community settings

  • Practical tasks such as cooking, budgeting, organisation and transport participation

  • Occupational therapy assessment and structured screening tools

  • Self-report from the young person

  • Carer or residential staff input

  • Collaboration with caseworkers and multidisciplinary teams

This holistic approach helps ensure assessment findings reflect how skills are used in everyday environments.

What the Assessment Includes

Assessment domains include:

  • Daily living and self-care skills

  • Home management and life skills

  • Executive functioning and planning

  • Community safety and participation

  • Social skills and communication

  • Readiness for increasing independence

Detailed Report and Readiness Profile

Following assessment, a detailed report is provided outlining:

  • A readiness profile highlighting current functional strengths and areas where further skill development or environmental support may be beneficial

  • Recommendations for therapy, support services or independence planning

  • Information to assist care teams with transition decision-making

  • Depending on the young person’s capacity and preferences, a youth-friendly summary report may also be created to support their understanding and engagement in planning.

Where appropriate, young people may access ongoing occupational therapy support following assessment, either through individual intervention or targeted group programs focused on building independence skills.

Do I need a referral?

You do not require a referral for services at Balance to Bloom.

Dependent on your funding source, information from your GP may be required. Refer your child or family for our services here.

Does my child need a diagnosis?

No, at Balance to Bloom we work with children and families for several reasons. Whether your child is yet to be diagnosed, has a diagnosis, or just needs a helping hand, we are here for you.

What does our process look like?

Balance to Bloom was born out of the need to personalised, family-centred services. Our initial processes include information gathering, collaborative goal setting, and assessment followed by therapeutic intervention. For more information, we have put together a short document that outlines our process and costs attached.

What funding sources can I use?

Clients privately funding therapy may be able to partially fund up to 5 sessions through Medicare, in consultation with their GP, or through private health insurance. Balance to Bloom accepts clients through NDIS who are self- or plan-managed.
